The NEW 2024 Sports Physical Packet is NOW AVAILABLE on the PHS Athletics Page! This Annual form is required by the NJSIAA to ensure our athletes are at their personal best.
The Palmyra School District School Doctor will be performing FREE PHYSICALS again this year on the following dates: May 31st, June 3rd & June 5th. These physicals will take place here on campus in the PHS Nurse's Office. All school sports programs for MIDDLE & HIGH Schools require them including Cheerleading, Marching Band & Color Guard.
You are welcome to have your family doctor complete these forms, but if you wish to have the school doctor complete the physical for your child, you must return the entire packet to the Main Office OR Nurse's Office BEFORE May 10th in order to be scheduled.
It is IMPORTANT to NOTE: MANY PAGES of these forms require signatures from both the ATHLETES & their parent/guardians. The packet MUST BE COMPLETED in its entirety, please be as neat as possible.
